If you have a problem with your health care or have had a negative experience when visiting our practice, we would like to know. Please feel free to discuss any problems you may have with our Practice Manager. Alternatively you may want to ask our reception staff for a complaint form and hand it back to us in person or by post. All complaints are taken seriously and you may remain anonymous if you wish.

Our practice is committed to providing you with the best preventative care possible. We may issue you with a reminder notice on some occasions. We also participate in the state/territory reminder systems. If you do not wish to be part of this system please let your doctor or reception staff know.

We collect information necessary to provide you with medical care. This may include your name, address, date of birth, gender, health information and family history, and contact details. This information may be stored on our computer system and/or in handwritten medical records.
Wherever practicable we will collect information from you personally. However, we may also need to collect information from other sources such as treating specialists, radiologists, pathologists, hospitals and other health care providers.
In emergencies we may also need to collect information from your relatives or friends.
By law we may be required to retain medical records for certain periods of time, depending on your age.
